As someone who's had chronic back pain, plus multiple stints in physical therapy, I know firsthand how the wrong mattress can exacerbate stiffness and discomfort. While no mattress will cure your back pain, a good one will cushion your pressure points without going against the natural curve of your body. That’s why it’s important to focus on three key factors: pressure relief, proper support, and healthy spinal alignment.

Our top picks:

As a scientist in the Good Housekeeping Institute Textiles Lab, I regularly evaluate mattresses, assessing everything from their materials and construction to their real-world performance. To find the best mattresses for back pain, I analyzed survey data from more than 10,000 mattress owners and worked with hundreds of at-home consumer testers to review over 250 options, prioritizing those that earned high marks from users with chronic back and hip pain. The Nolah Original mattress has earned our top spot several years running.

Pros

  • Pressure-relieving, responsive foam
  • High praise from testers with joint pain
  • Sleeps cooler than other foam beds we've tested

Cons

  • Low height may be unideal for those with mobility issues

WHO IT'S BEST FOR: People with chronic pain (including neck, shoulder, back, and hip pain) in search of a pressure-relieving bed that won't compromise on responsive support

WHO SHOULD AVOID IT: Stomach sleepers, people with large frames who need a firmer bed, and those who aren't keen on the idea of an all-foam bed

This mattress consistently earns unanimous praise from testers with back or hip pain. Its comfortable medium level of firmness cushions your pain points without sinking too much under your weight. We also like that there's a layer of zoned foam at the top to promote proper spinal alignment.

✔️ HOW IT COMPARES: Nolah injects its foam with extra air for a more responsive texture than memory foam, but it's still just as pressure-relieving. The first time I tried this mattress, I felt like a weight had been instantly lifted off my hips and shoulders. Our consumer testers agreed: One liked how it "offers support while still cradling pressure points."

HOW IT PERFORMED IN OUR TESTS:

✔️ Excellent user feedback: Our survey respondents and at-home testers—including those with medical conditions like sciatica and scoliosis—highlighted its balanced feel. Several said that it alleviated both their back and hip pain. One shared that it was "significantly helpful in relieving back pain for [her] husband who has scoliosis."

✔️ Temperature: Foam is notorious for overheating, but survey panelists told us this one sleeps cool compared to other mattresses (foam and non-foam) they've owned.

✔️ Height: Our testers were surprised by how comfortable this slim mattress felt. The caveat is that the 10-inch height may be too low for those with mobility issues, depending on the height of your bed frame.

Luxe Hybrid Mattress

Pros

  • Great feedback from testers and survey panelists
  • Bargain price for a hybrid mattress
  • Fast shipping

Cons

  • Some said it lacks edge support

WHO IT'S BEST FOR: Price-conscious shoppers looking for an affordable bed that cushions their pressure points without skimping on support will appreciate this model. In our tests, it performed best with young adults (including those with back or hip pain).

WHO SHOULD AVOID IT: Older users or those with limited mobility may find the edge support and DIY setup less than ideal.

If you're on a budget, I'd recommend the Allswell Luxe Hybrid. It costs much less than our other top-tested picks yet still gets glowing reviews from testers and panelists for its comfort and support. Several even compared it to a high-end hotel bed, commending the high-quality feel in spite of the low price. Walmart also offers fast shipping.

✔️ HOW IT COMPARES: This hybrid gently contours to your body, but you'll still feel like you're sleeping "on top of" the mattress rather than "in" it. You can pick between a versatile Medium level of firmness or an Extra Firm, which is designed for users with larger frames.

HOW IT PERFORMED IN OUR TESTS:

✔️ Excellent value: Hybrid mattresses (like this one) are the most popular in our tests, but they're often pricey: The average cost of a Queen-size hybrid mattress was $2,050 in 2023. We like that this model strikes a nice balance between the cushy foam and coils. The foam cushions pain points while minimizing motion transfer, and the coils add breathable support.

✔️ Comfort and support: Overall, it earned high scores for its reliable support and pillowy top. Some panelists with back pain highlighted its comfort: "It's great for my back," said one. One tester also said, "I wake up feeling refreshed."

✔️ Edge support: The edges sunk down more than other mattresses we've evaluated, which can make getting in and out of bed a little more challenging.

Posturepedic Spring Mattress
Credit: Good Housekeeping/Mike Garten

Pros

  • Budget-friendly price
  • Innovative coil design for more support
  • Includes white-glove delivery

Cons

  • Can feel hot, according to some testers

WHO IT'S BEST FOR: Those looking for a no-frills mattress with tailored support that doesn't break the bank

WHO SHOULD AVOID IT: Anyone who prefers a cushier, foam-forward feel

Last year, Sealy upgraded its popular Posturepedic line with innovative coils for maximum support and tailored comfort. Its entry innerspring model was a favorite in our tests for its versatile feel. Plus, it's delivered straight into your home with no extra cost.

✔️ HOW IT COMPARES: The unique coils provide Goldilocks-level comfort and support—not too soft, not too hard—which is a difficult feat for an innerspring mattress. The Medium firmness makes it an approachable option for a wide range of sleepers, though some may prefer the Soft or Firm variations that are sold exclusively at Mattress Firm.

HOW IT PERFORMED IN OUR TESTS:

✔️ Standout coils: The new coils are truly one-of-a-kind, having a tapered design with a tightly-wound top and a looser bottom to mimic pricey micro-coils. This helps it lightly conform to users with smaller and lighter frames or provide deeper, firmer support for those with larger or heavier frames. Beyond the unique shape of the coils, the pocketed coils have a zoned layout, with a firmer section around the lower back that users appreciated.

✔️ Comfortable support: One tester noted, "It was firm enough to not sink into yet had a lot of cushion to it." Another liked that it evenly distributed her weight and eased her pressure points. Testers also agreed that it was easy to transition to the bed and reported better sleep. One said, "In the mornings, I feel well rested and don't feel stiff at all."

✔️ Cooling: While innerspring mattresses sleep cooler than foam, some hot sleepers noted that this one ran warm compared to their previous mattresses, while others didn't have any issues. Still, we like that the cover is moisture-wicking.

Bear Elite Hybrid

Pros

  • Targeted zones for spine alignment
  • Perfect comfort and support ratings from every tester
  • Testers said it improved their sleep quality

Cons

  • Outer cover pilled over time

WHO IT'S BEST FOR: Anyone interested in a balance of tailored relief and support

WHO SHOULD AVOID IT: Those looking for plush pressure relief or extra firm support are better off with all-foam or innerspring mattresses.

Hybrid mattresses are designed to combine foam and coils for a happy medium, but few can hold a candle to the Bear Elite Hybrid when it comes to comfort and support. It's garnered glowing praise from all of our testers, including several with recurring back pain.

✔️ HOW IT COMPARES: Bear's seven-zone layout offers more give or support in key areas, like the shoulders and hips, to promote spinal alignment. It's earned unanimous praise from every tester, including Lexie Sachs, GH's executive director of testing strategy & operations, who's been testing mattresses for over 15 years. She found herself sleeping through her alarms for the first time in years within a month of sleeping on the Elite Hybrid.

HOW IT PERFORMED IN OUR TESTS:

✔️ Zoned support: Most mattresses with zoned support use three sections with firmer coils around the lumbar region, so Bear's seven-zone design feels more fine-tuned. When I first laid down on this mattress, I was surprised by how instantly comfortable I felt.

✔️ High praise from testers: All our sleep testers have reported improved sleep quality. A recent tester with chronic pain shared, "The pain in my shoulders or hips that would usually creep in when trying to fall asleep, or eventually wake me up in the middle of the night, has subsided dramatically." Others shared that they wake up feeling well-rested.

✔️ Durability: While it's on the newer side (it was first launched in 2022), it has held up well in our tests so far, aside from some pilling on the outer cover after the two-year point.

Note: Bear makes some claims on its product listing that we weren't able to verify, specifically about how Celliant fibers use infrared technology to promote recovery, but we still stand by its comfort.

Best for Side Sleepers

Tempur-Pedic Tempur-Cloud

Best for Side Sleepers

Tempur-Pedic Tempur-Cloud

Credit: Good Housekeeping/Mike Garten

Pros

  • Tempur-Pedic foam at a lower price
  • Adapts to your body shape for personalized pressure relief
  • Great motion isolation, according to testers

Cons

  • Foam may run warm

WHO IT'S BEST FOR: Side sleepers looking for personalized support for their hips and shoulders or back sleepers who like the feel of foam

WHO SHOULD AVOID IT: Those who prefer a firmer surface over the sinking-in feel of memory foam and hot sleepers since this pick ran warm in our tests

No other mattress we've tested provides precise pressure relief and support like Tempur-Pedic. The Tempur-Cloud delivers superior motion isolation and contouring foam at a more affordable price than the brand's other non-boxed foam models.

✔️ HOW IT COMPARES: Compared to other all-foam beds, it feels more luxurious, from the cloud-like texture to how the foam is perfectly tailored to the shape of your body. It does sink in as it adjusts to you curves, but you'll feel firmly cradled for maximum relief. One tester said it best: "You really feel hugged by the mattress."

HOW IT PERFORMED IN OUR TESTS:

✔️ Precise support: Our testers confirmed that it quickly molds to your body for precise support, though it can feel a bit hard when you first lie down. One combination sleeper said, "I feel like my entire body is supported in a very gentle way."

✔️ Pressure relief: Our most recent testers were blown away by just how comfortable they felt sleeping on this bed. Side sleepers in particular praised how well the foam cradled their hips and shoulders. This is especially important for maintaining a neutral spinal alignment: Sleeping on your side may relieve back pain, but you may also experience hip or shoulder pain if sleeping on a too firm bed.

✔️ Motion isolation: Testers who share the bed with partners or pets were impressed with the motion isolation and reported improved, uninterrupted sleep.

✔️ Temperature: The downside of memory foam is that it traps heat, so hot sleepers who love the feel should consider a cooling mattress, like one from the brand's Breeze line.

Classic Mattress

Credit: Good Housekeeping/Mike Garten

Pros

  • Good edge support
  • Two coils bases for added breathability
  • Free setup and mattress removal

Cons

  • Runs firm, according to testers

WHO IT'S BEST FOR: People looking for support above all else; this responsive pick has breathable, sturdy coils and a reinforced lumbar region

WHO SHOULD AVOID IT: Those who prefer the cradling feel of memory foam; many told us that this mattress ran firm

Our testers who struggle with on-and-off back pain usually fall into two camps: Either they need gentle pressure relief or extra support on the lower back. While boxed mattresses stand out for their convenient shipping and cushioning feel, the Saatva Classic is one of the most supportive mattresses we've tested.

✔️ HOW IT COMPARES: It uses two layers of durable, low-gauge steel coils that can't be compressed like your typical bed-in-a-box. There's also extra padding in the lumbar region that hundreds of our survey respondents swear by. Still, the mattress itself runs firm as a result, so I wouldn't recommend it to anyone who prefers a softer bed.

HOW IT PERFORMED IN OUR TESTS:

✔️ Excellent support: One tester said, "I tend to have chronic back pain, which has much improved since using this mattress." The edge support is also impressive and makes it easier to get in and out of bed.

✔️ Breathable design: This mattress, while traditional, has nice features for hot sleepers: It uses two coil bases for optimal ventilation and minimal foam.

✔️ Excellent customer service: Our panelists consistently highlight the brand's customer service, especially the short wait times. We like that the brand also offers free white-glove delivery and mattress removal with a generous 365-day trial, though you'll have to pay a $99 return fee if it's not the right fit.

Purple Mattress

Credit: Good Housekeeping/Mike Garten

Pros

  • Excellent breathability, according to testers
  • Pressure relief without feeling too soft
  • Grid offers motion isolation

Cons

  • Shortest mattress in our list

WHO IT'S BEST FOR: Hot sleepers in search of a more breathable mattress or those who want a firmer and more responsive mattress that still offers a cradling feel

WHO SHOULD AVOID IT: Cold sleepers or anyone who prefers a tall mattress (this is our shortest pick at 9.25 inches); those who prefer to use a mattress pad or topper won't feel the grid's cooling effects

The Purple Mattress is the most unique mattress on our list, and one of our favorites for its exceptional pressure relief and breathability, which was a highlight amongst many survey respondents. The topmost layer is made of an elastic polymer (in the shape of a 3D grid) that cradles your body where you need it most.

✔️ HOW IT COMPARES: Purple's GelFlex Grid is one-of-a-kind. It has a squishy texture and each square of the grid responds precisely to your body. As a result, you'll get the cradling feel and motion isolation you'd expect from memory foam with the springy, responsive nature of latex.

HOW IT PERFORMED IN OUR TESTS:

✔️ Excellent breathability: Aside from its unique feel, the grid offers superior breathability. Some of our testers even found the mattress too cool during the winter.

✔️ Pressure relief: The GelFlex Grid stands out for how it relieves pressure from pain points without sinking in like foam. Many of our testers and panelists appreciated its ability to relieve their and their partner's back pain. One said, "This is the first mattress where we both woke with no pain." Another added, "It only took a couple of nights of sleeping on the Purple mattress for my back to feel much better."

✔️ Height: Some of our testers found it a bit too short for comfort, given its low 9.25-inch profile. If you prefer a taller, hybrid mattress or are willing to splurge, Purple's RestorePlus Hybrid is designed with additional lumbar support and a medium-soft feel.

Pros

  • GOTS and GOLS certified
  • Latex and coils for comfort and support
  • Year-long trial period

Cons

  • Medium and Plush models cost extra

WHO IT'S BEST FOR: Eco-conscious consumers who prefer organic materials and shoppers looking for a more responsive bed

WHO SHOULD AVOID IT: Sleepers who prefer the conforming nature of memory foam—while latex is excellent at contouring to your body, it doesn't offer the cradling feel of foam.

This certified-organic mattress is stacked with supportive features designed to promote spinal alignment. The coil base has various zones with extra give or support, while the latex foam works to evenly distribute your body weight without sinking in. It comes in three firmness levels, though you'll need to pay a little extra for a softer feel. There's also a 365-day trial period.

✔️ HOW IT COMPARES: Latex feels very different than foam or coils. It's notably springy and can feel both dense and airy at the same time. While it will contour to your body, it also pushes against your body for a "floating" sensation. The Avocado Green Mattress balances that responsive latex with zoned coils for a premium feel.

HOW IT PERFORMED IN OUR TESTS:

✔️ Certified organic: Latex is responsive yet pricey, so you'll want to make sure it's high quality. Luckily, this mattress is fully certified by GOTS (Global Organic Textiles Standard) and GOLS (Global Organic Latex Standard), so you know the entire manufacturing process follows strict guidelines.

✔️ Comfortable & firm support: The bed has a responsive feel from the latex, so you feel like you're floating on air rather than lying down on a cushy surface. Beyond that, the zoned coils provide extra support near the lower back, which our testers greatly appreciated. One said, "The bed definitely helped with my upper back and lower back pain, and when I go to bed with hip pain, I wake up refreshed in the morning."

✔️ Durability: Many survey respondents told us their beds have held up well over the years without developing any dips or indents. One told us, "It's held up even after two years, when other expensive mattresses we've purchased were already showing signs that they'd need to be replaced."

Which type of mattress is best for back pain?

double line break

Memory foam mattresses and hybrid mattresses are both great options when you’re shopping for a mattress and you have back pain, according to Jaspal R. Singh, M.D., co-director of the Center for Comprehensive Spine Care at Weill Cornell Medicine and New York-Presbyterian. There isn’t one definitive type of mattress that’s best for back pain, but patients who sleep on a mattress with some type of memory foam often report a more supportive and restful sleep, since memory foam offers both back support and pressure relief, especially around the hips and shoulders.

For those who don't like memory foam, don't worry. While it certainly offers benefits, memory foam isn't necessarily superior to other types of mattresses like innerspring or latex, which are known for their responsive support. "Patients should select mattresses that provide adequate support to their back. Memory foam vs. traditional is a matter of individual preference. Medically, one is not superior to the other," says Amit Jain, M.D., M.B.A., chief of minimally-invasive spine surgery and associate professor of orthopedic surgery at Johns Hopkins University School of Medicine.

Are mattresses with “zoned support” better for back pain?

double line break

If the back pain you have is mechanical (the most common form of back pain and the result of a strain or injury), Dr. Singh says that seeking out a mattress with zoned support may be worth it. “The human body does not load the mattress evenly,” he says, explaining that the pelvis and shoulders generate more force than the lumbar spine. Zoned support can help maintain spinal alignment and prevent sinking of the hips and shoulders.

Dr. Jain also says that mattresses with zoned support may be helpful for people with back pain. “Zoned support mattresses may help maintain spinal alignment by providing varying stiffness across body regions to match weight distribution and body contour,” he says.

What firmness level is best for relieving back pain?

linebreak

Medium firmness is effective in improving pain, according to Naimish Baxi, M.D., a physiatrist at the Hospital for Special Surgery, as well as a recent review of many medical studies. Plus, medium firmness offers an ideal balance of cushioning and support for side sleepers. A too-firm mattress doesn't let your shoulders or hips sink into the mattress, causing inadequate support of the neck and shoulders. On the other hand, if it’s too soft, there won’t be enough support, causing the hips and shoulders to sink in too much, resulting in improper spinal alignment.

What features in a mattress help with chronic versus occasional back pain?

linebreak

How often you’re experiencing back pain matters when it comes to choosing a mattress. Dr. Singh says that people with chronic back pain who need more consistent spine balance or pressure distribution will likely benefit from a mattress with Medium-Firm support.

For occasional back pain, he says that temporary contouring mattresses, such as memory foam, may be more helpful.

What is the best sleep position for back pain?

double line break

Side sleeping is often recommended if you have back pain, especially if you have pre-existing back problems. Dr. Singh even recommends side sleeping with a pillow between your knees, which will help take pressure off the lumbar spine.

That being said, the most important thing to do for back pain while sleeping is to keep your spine in neutral alignment. If you have lower back pain, sleeping on your back can “help alleviate pressure through the lower lumbar discs,” says Dr. Baxi. He also warns, “Sleeping on your stomach is generally not ideal due to the poor spine alignment when turning your head to one side.

What is spinal alignment and why does it matter?

double line break

"Spinal alignment refers to the ability of the head to be spatially located on top of the pelvis to minimize the energy expenditure for the body," says Dr. Jain. A neutral spinal alignment can be vital, especially for those with existing pain. If sleepers spend too much energy going against the natural curve of their body, that effort can result in additional back pain. Similarly, it's crucial that your mattress and pillow accommodate the position of your spine, providing just enough comfort and support to maintain it.

Not sure if your spine is aligned correctly? Dr. Singh recommends this test to check: “If you are sleeping on your side, make sure that your nose, chest, and pubic bone fall in approximately the same line. Your shoulders and pelvis should sink into the mattress somewhat, but not to the point where you’re forming a C-shape into the mattress,” he says.

Prefer back sleeping? Jonathan S. Kirschner, M.D., RMSK, an interventional spine and sports medicine specialist physician, says that a good sign is feeling comfortable without pressure points or pain. Dr. Singh says this too, adding that tossing and turning is a sign that you might be sleeping with your spine out of alignment. Another major sign he says to watch out for: morning stiffness.

What is the best type of pillow to help with spinal alignment?

linebreak

Picking the right mattress is only one part of supporting your back while you sleep; the type of pillow you use is important too. For side sleepers, Dr. Singh says that a firmer, higher loft pillow that fills the distance between the mattress and ear is beneficial.

For back sleepers, he says that having a medium loft pillow (a pillow with a thickness height of between three and five inches) that supports the natural spine curve of the neck without pushing the head forward is best.

“Stomach sleepers have the biggest challenge because any stress is unavoidable, but a low loft or a soft down pillow can be beneficial,” he adds.

Can a mattress actually get rid of back pain?

double line break

It depends. Chances are, your pain is caused by an underlying issue beyond your mattress, yet buying the right mattress can certainly be one factor in eliminating your back pain.

Dr. Baxi says, “Back pain can be complicated and has many other contributing issues. For someone with chronic lower back pain, mattress characteristics can contribute to reduced pain throughout the day." In fact, he said some studies have shown that the right mattress can improve pain, stiffness, and sleep quality by up to 60%. For individuals with frequent neck and upper back pain, the height of your pillow may be another factor to consider.

That being said, it's a good idea to consider what else can be causing your pain. “It’s mostly related to what we do during the day that dictates the impact on our spine,” says Dr. Singh. He adds that one example is sitting at a desk all day, which puts pressure on the back and can cause lower back and neck pain.

If you find that you're experiencing recurring pain upon waking, it's best to consult with a physician to identify the nature of the pain, as a new mattress may not fully address any pre-existing conditions.

How long should I try a mattress before deciding if it helps my back pain?

linebreak

Even if your first night on a new mattress is the best sleep of your life, all the experts we talked to say it takes some time to determine just how much a mattress is helping back pain, so it’s worth choosing a mattress with a free trial period or money-back guarantee.

Dr. Jain says that, ideally, it’s best to test a new mattress for 12 weeks before keeping it for good, but he says the minimum should be a four-week trial. “Studies assessing mattress effects on back pain use trial periods of 28 days to 12 weeks, with significant improvements in pain and disability typically observed by four weeks and continuing progressively through 90 days,” he says.

A mattress trial period is especially important for someone with back pain who struggles to find a comfortable bed. Many brands offer returns for at least 100 days so you can sleep on the mattress for a few months and get a refund if it's not a good fit. All our recommendations have several-month-long trial periods.

Can a mattress fix posture issues?

linebreak

All the experts we talked to said that even the best mattress can’t fix posture issues. Dr. Singh says that posture is primarily governed by muscle endurance, mobility, flexibility, and whether a neutral spine is maintained while doing daily activities. “Improving your sleep hygiene and mattress may reduce aggravation, but it does not replace rehabilitation,” he says.

Dr. Jain agrees, adding, “Mattresses cannot directly correct posture issues, but Medium-Firm mattresses can improve pain and disability in patients with chronic low back pain, by optimizing pressure distribution and muscular function during the approximately one-third of life spent sleeping."

The competition

double line break

We've tested hundreds of mattresses, but not all made our list of the best mattresses for back pain. Still, here are a few popular options you may be curious about:

  • The Saatva Rx is a popular launch, but it earned mixed reviews from consumer testers with back pain. Though it felt soft in the store, one tester found it ran too firm for comfort. We're currently in the process of re-testing this model and will report back.
  • Nolah's Evolution is the brand's most popular mattress, but at 15 inches, it can be tough to set up. It's received good feedback from many, but some testers found it ran soft. The Original ultimately garnered better and more consistent feedback over time.
  • The Winkbed is another popular hybrid mattress, but it's received mixed reviews in our surveys and tests. Our most recent tester found the support solid and uniform, but remarked that it slept notably hot and ran firm, ultimately aggravating her shoulder and lower back over time.
