Heads up if you use GLP-1 injections, especially ones from compounding pharmacies. On September 24, Apothecary Pharma LLC of Cary, North Carolina, initiated a voluntary nationwide recall of its tirzepatide injections. (Tirzepatide is the active ingredient in the GLP-1 medications Mounjaro and Zepbound.)
The Apothecary Pharma LLC is a compounding pharmacy, so it doesn’t actually manufacture those brand-name drugs. Instead, a compounding pharmacy mixes, alters, or combines the ingredients in medication to create a new customized product. Compounded medications are also not approved by the U.S. Food and Drug Administration (FDA).
Why were the GLP-1 medications recalled?
The reason for the recall was “lack of sterility” of the injections. On October 9, the U.S. Food and Drug Administration (FDA) issued an enforcement report that gave the recall a Class II designation. This means that if you use or are exposed to a recalled product, you could experience “temporary or medically reversible” adverse health effects, but it’s unlikely that you would suffer serious health problems.
The FDA had previously sent a warning letter to Apothecary Pharma LLC in December 2026, outlining a number of violations in the pharmacy’s compounding process. One violation the FDA addressed then was a lack of sterility in its drug production process.
What GLP-1 injectables were recalled?
There are two specific products listed in the FDA’s enforcement report. Check the medication you have for the following information:

What should you do if you have the recalled medication?
The FDA report didn’t provide any instructions as to what to do with the recalled medications, and the company hasn’t offered any consumer guidance. Contact your health care provider immediately to find out what action is specifically right for you, in terms of guidance about discontinuing or changing your GLP-1 medication.
